About Connecting for Better Health

Connecting for Better Health (C4BH) is a nonprofit coalition dedicated to advancing health and social service data sharing in California. The organization aims to improve the health of all Californians by ensuring that care teams have the necessary information for seamless, high-quality, and affordable care. C4BH collaborates with providers, caregivers, health plans, advocates, innovators, and community organizations to discuss developments, share innovations, and advocate for better data sharing policies.

Data Exchange Framework (DxF) Support:C4BH supports California's Data Exchange Framework, a statewide data sharing agreement to facilitate secure exchange of health and social services information among health care entities, government agencies, and social service organizations for improved whole-person care. Services include education, technical assistance, policy monitoring, and advocacy.

DxF Community Sandbox:A dynamic learning environment that allows organizations to securely test, collaborate, and scale new workflows to support California's Data Exchange Framework.

Community Design Studio:A platform for community collaboration on data sharing use cases, such as housing referrals, to enhance health and social service delivery.

Resources and Newsletters:C4BH provides resources, blogs, newsletters, and meeting materials to educate and inform stakeholders about data sharing innovations and policies.
